How about writing about your biggest fear?
for example: expiriencing the sharp pain you get at the dentist every time you masturba----- I mean, every time you eats sweets :p and for one to get an orgasm you have to pull out a tooth ... ok, random thoughts shouldn´t be written down, let´s leave it at something more normal and typical:
